MHAA Procedure for Micro-chipping

Due to requirements for registration
contained in the MHAA Constitution the
following procedure applies as of 1/1/06:
Obtain the official MHAA Microchip Request Form from the National Office or download it from the forms page. You will need a separate form for each horse you are microchipping
Contact your Vet and arrange for a universal microchip to be implanted and for the VET to complete all details indicated on the Microchip Registration Form before signing.
Make a photocopy of the signed Microchip Registration Form and registration application before posting both back to National Office. This form, when received will confirm that the microchip has actually been implanted thus identifying your horse and registration will be processed and marked accordingly
